INSURANCE CLAIMS:
If you are using medical insurance for your visit, you must provide a copy of your insurance card and a state ID to complete the insurance verification prior to your appointment. Please email documents to [email protected] to have your benefits verified. A deposit of $90.00 will be collected at time of service for benefits that have not been verified prior to the appointment. We will contact you by phone or email once we have completed the verification.
If you are seeking treatment for Motor Vehicle Accident or Work Injury you MUST have a prescription from a doctor for treatment prior to receiving treatment. They will provide a script with your diagnosis codes and the frequency of your treatment.
Your PIP (Personal Injury Protection) will not cover services without a script. This is very important, failure to provide a script will result in cancellation of your future appointments and you may be responsible for the full amount of the billed services.

The max allowed treatment time for plans cover up to 60 minutes of massage therapy. If you are a new client we require pre-verification of benefits.
